
We are trying to improve the way we register within this practice. As a new patient you will be given a number of forms to fill in. These forms MUST be completed before you can be fully registered, failure to do so may result in you being refused registration on that day.
Patients transferring from another surgery
Name and address of your previous GP
NHS number if possible
Proof of address for the address you are registering from (This can be a bill, bank statement, tenancy agreement, or letter from your letting agency)
Photographic ID
For children: a record of all previous immunisations received by your child
Patients registering with the NHS for the first time (From another Country)
If you are newly arrived into the UK for a period of no less than six months, you may register as a patient with the NHS. You will need to provide the following information.
Your passport
Your visa, European Insurance card (EH1C) or letter from the Home Office/Consulate authorising your stay in this country.
Proof of address for the address you are registering from (This can be a bill, bank statement, tenancy agreement, or letter from your letting agency)
For children: a record of all previous
immunisations received by your child
